# Open vs Closed Pyometra: Why Emergency Surgery Is Urgent

Open vs closed pyometra in dogs: what each means, why closed cases are more dangerous, and why surgery beats medication, for Talleyville owners.

## Same disease, two very different presentations

**Pyometra is a bacterial infection that fills the uterus of an unspayed female dog with pus, usually two to eight weeks after a season. “Open” and “closed” describe whether the cervix is open, letting pus drain out, or sealed, trapping it inside.**

That one anatomical detail changes how the disease looks, how quickly it becomes critical, and how likely an owner is to spot it in time. Our

sees both forms in Talleyville dogs, and the closed cases consistently arrive later and sicker.

## Open pyometra

The cervix stays open, so infected material drains out through the vulva. Owners notice a foul-smelling discharge, sometimes bloody, sometimes creamy, and usually on the dog’s bed or a blanket before anywhere else.

It is unpleasant, and it is also genuinely useful. The discharge is an unmistakable warning sign, and it relieves some of the pressure building inside the uterus. Open cases are usually seen earlier and tend to be less critically ill on arrival.

They are still an emergency. Open pyometra progresses to sepsis just like the closed form. It simply gives you more warning.

## Closed pyometra

The cervix is sealed. Nothing drains, nothing smells, and there is nothing on the bedding.

Pus keeps building up inside a uterus that cannot empty. Pressure rises, the uterine wall thins and stretches, and rupture becomes a real risk. If the uterus ruptures, infected material spills into the abdomen and causes peritonitis, which is far worse than the pyometra itself.

Meanwhile the dog’s only signs are general ones: lethargy, going off her food, drinking and urinating more, and sometimes vomiting. They are vague and easy to put down to something minor, so owners understandably wait a day or two.

> **The absence of discharge is not reassurance. It is a reason to look harder.**

## Why we do not attempt medical management

Prostaglandin treatment exists. It works by making the uterus contract to push out the infected contents, and it is occasionally used in valuable breeding females with open pyometra who are otherwise stable.

The reasons our surgeons recommend surgery instead in almost every case:

- It is **unreliable**, failing often enough that time is lost while the dog deteriorates
- It is **slow**, taking days during which the infection continues
- It is **inappropriate for closed pyometra**, where forcing contractions against a sealed cervix risks rupture
- It has a **high recurrence rate** at the next season, because the underlying endometrial hyperplasia remains
- It causes **significant side effects**, including vomiting, restlessness, and abdominal pain

Surgery removes the source of infection completely and permanently, in a single procedure, with a rapid improvement in most dogs.

## What the surgery involves

Stabilization comes first: IV fluid resuscitation and broad-spectrum antibiotics, with bloodwork to establish packed cell volume, total protein, and kidney values before anesthesia.

Then an ovariohysterectomy, removing both ovaries and the entire infected uterus. Unlike a routine spay, the uterus is heavy, fragile, and full of pus, so it has to be lifted out without tearing. The uterine stump is ligated with great care, because a leaking stump is the main complication afterwards. If there has been any contamination, the abdomen is flushed before closure.

Throughout, continuous monitoring tracks blood oxygen, ECG, blood pressure, and temperature. Septic dogs are prone to low blood pressure under anesthesia, and that is the number that matters most.

## Afterwards

Most dogs look dramatically better within 24 hours of the infected uterus being removed. Recovery takes about two weeks of restricted activity, plus a full course of antibiotics.

Pyometra cannot come back, because the uterus is gone. Surgery through Talleyville Dog Surgery starts at $1,100, and urgent cases are booked same or next day.

Guide FAQ

## Questions owners ask

01Why is closed pyometra more dangerous?

Because the pus has nowhere to go. With the cervix sealed, infected material builds up under rising pressure inside the uterus, increasing the risk of rupture into the abdomen. There is also no discharge to alert the owner, so closed cases usually arrive later, when the dog is already seriously ill.

02How quickly do I need to act?

Immediately. Pyometra is a surgical emergency that progresses over hours rather than days. Once it is suspected, your dog should be assessed and stabilized the same day, with surgery as soon as she is fit for anesthesia.

03What does pyometra surgery involve?

An emergency ovariohysterectomy, removing the whole infected uterus and both ovaries, supported by IV fluids and broad-spectrum antibiotics. Because the uterus is fragile and full of pus, the challenge is removing it without rupturing it.
